In August 1996, Congress passed the Health Insurance Portability and Accountability Act, also know as HIPAA. HIPAA consists of 9 components. They are Electronic Transaction & Code Sets, Privacy, Security, National Provider Identifier, National Employer Identifier, National Health Plan Identifier, Enforcement, Doctors First Notice of Injury, and Claims Attachment. In December 2000, the final privacy regulations were published. These regulations require us to tell you about your rights to your protected health information (PHI) and to educate our staff about confidentiality and security of PHI.

First, just what is PHI? PHI is any information recorded in any medium, including oral, written and/or electronic, which relates to the past, present, and future treatment of an individual. Protected health information is any information that is sufficient enough to identify the individual.

Health information about you may be used and disclosed by The Pottsville Hospital and Warne Clinic to carry out treatment, payment, or health care operations. Health care operations are activities undertaken for the purpose of carrying out the management functions of the hospital or necessary for support of treatment or payment.
